Good Faith Estimate Notice

Under the No Surprises Act, patients who are uninsured or choose not to use their health insurance have the right to receive a Good Faith Estimate of expected healthcare costs before receiving services.

A Good Faith Estimate explains the expected charges for scheduled healthcare services, including psychiatric evaluations, medication management appointments, and other applicable services.

If you schedule services with Fitsam Psychiatry PLLC and are paying out of pocket, you may request a Good Faith Estimate before your appointment.

If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges.

For more information about your rights under the No Surprises Act, visit the Centers for Medicare & Medicaid Services (CMS) or contact our office.
